this leadsheet had thinsculpting and another client's email on here as having access instead of Dr. Walter's email. I removed the bottom two emails. But did that happen when it was duplicated?

Vania - I always use the below blank template sheet while setting up a new client leadsheet. We don't have either of those two emails in that blank template sheet.
So it couldn't be because of this reason.

Isaac  thats SO weird then I wonder how these two random clients ended up there?

Vania, the only logical explanation is that I may have added them by mistake. Unless someone else did.
In any event, glad you caught it so we are good to go.

Thank you for sharing that template. I made some changes to it that I had to keep doing to the leadsheets each time you created a new one. 
1. - Changed the top row to Deep blue, letters to white. 
2. - Froze the top row.
3. - Made the phone column CENTER aligned so phone numbers come out even.
