Patient comes in for an evaluation and is seen by Dr. Altman personally. They are examined. Desires are established. Reasonable expectations are set, good size and fit for the patient, both for aesthetics and for life. Consultations take up to 1 hour. 97% closure rate of people that come in and go through with the surgery. 

Day of surgery Surgery - discuss size again, go over implant size. In the operating room, Dr. Altman positions client in a way that their arms are down by the side, this makes it less painful. Typically, doctors have women's arms outstretched in a 90-degree angle. This position creates more tension and leads to more pain. With the arms relaxed down by the side, the muscle is relaxed. By using this position, the procedure is more comfortable and less to no post-operative discomfort is experienced. Dr. Altman coined his surgery as the no-downtime breast augmentation. 

Always use a dual-plane technique. The breast tissue is elevated for a short distance before creating the submuscular pocket for the implant. This enables the breast to be more full at the bottom than at the top. This results in a much more natural appearance and feeling. 

Once the pocket is created, the implant is placed using a no-touch technique. This results in less risk of infection or development of scar tissue (capsular contracture). After the procedure, the incision is meticulously closed using two layers of absorbable suture and then covered with a surgical adhesive so that the patient can shower within 24 hours. This is called the no-downtime breast augmentation because you can wear your normal underwire bra within 24 hours, return to work within 48 hours (for non-physical job), and there is no need for narcotic pain medication. The actual surgery (or procedure itself) lasts 30 to 45 minutes. This means less anesthesia, less chance of bleeding, and less pain. Ultimately less grogginess and fatigue throughout the remainder of the day.  Mild swelling will usually resolve within 1 to 2 weeks. Stitches are absorbable. If you work out regularly, most patients can resume normal exercise within 2 weeks, however they cannot perform chest exercises until cleared by Dr. Altman.
