  Hector Mota, Operations Manager at Medstar Hector -  The GHL forms has been set up and connected to the leadsheet now. I have connected them to the same sheet but in a different worksheet.

Currently there is an issue with the GHL form not passing source URL info. I have already created a ticket with their support team regarding this.
