Will remove the old ZAP integration from all forms now. - DONE

- also create new lists in constant contact based on service type and one for general contact and route zaps there - This site doesn't have separate forms and using same sidebar form for almost all services on their website. So for now I have created list in Constant contact account based on the form type.

Below are the name of new email lists that I have created in Constant Contact -

1) Sidebar form list
2) Optin list
3) Consultation form list
4) Coolsculpting service list
5) Contact form list
