I am seeing that the side forms for this client are not connected to the leadsheet. 
Can you please check out what is going on?

Hector Mota, Operations Manager at Medstar Hector - I just tested it and test data came successfully in the sheet. Had to click on submit button thrice though before my form got submitted successfully. whitney.zelig@gmail.com, Medstar whitney.zelig@gmail.com has already made the client aware of this issue.

Good morning.
Thank you for checking this and the update.
So you having to hit the submit button 3 times is not an option for potential clients.
It seems we always have issues with sites that have 3rd party developers.

Okay so I tested this again and the pop up is working fine.
The side form took a few minutes to show up but it did populate.
