page and I am NOT seeing the pop-up. 
I checked on another computer, cleared out cookies and I am still not seeing a POP-UP. 
Can you please verify that this is working?

Without a POPUP on this page https://www.bellocorpomedspa.com/services/coolsculpting , there is really no way for a LEAD to fill out a form. There is NO STATIONARY form on this page. Shouldn't they have a Stationary form on that page anyway?

Hector  - make sure the site is marked no index in reading settings of wordpress. Until we launch his site completely we do not want him to get duplicate content. 

So I went into reading settings.. Search Engine Visibility | Search Engine Visibility = then ticked (checked off) Discourage search engines from indexing this site.
