Following the completion of client Chic Med Laser & Aesthetic Center - www.chicmed.com} and their website (https://chicmedlaser.com/) setup, began to check on pop ups, side forms and other forms to make sure they were working correctly. And that they were populating on the leadsheet. Found an issue with their Hydrafacial page, informed team to look into it. 

Isaac Klausnser, Medstar Isaac I am seeing that when I fill out the Hydrafacial  https://chicmedlaser.com/hydrafacial/ pop up on this LP, it comes out on the leadsheet as if I filled out the side form.
Can you please check to see why that is?

Hector Mota, Operations Manager at Medstar Hector - That's very strange. I just checked and all the settings looks good. I also did a test and seeing the correct test data received in the sheet. Can you please check again on your end? Let me know if it still doesn't show the correct information for you in the sheet.
