Can you have a look at the following for me?

1.) Update the Optimonster pop up with the image attached below.

2.) I was updating the Dr. Hill page and noticed that the mobile version of the SCHEDULE A YOUR CONSULTATION TODAY form indicates that we would send a $500 Gift Certificate towards surgical services with Dr. Hill. That’s really old. I deleted that form from the page as there is a form towards the footer that is also a schedule appointment form. Can you confirm that all the auto responder forms are no longer sending offers for Hill.

3.) I’m having trouble with the mobile responsive view. Desktop looks fine on the Dr. Hill page. But for some reason the second section, titled “Mobile — Offer Section” won’t display properly. The black background is white and some text displays while the top section doesn’t. It’s white on a white background. See the screen shots for details."
