Isaac  - lets make sure all forms on their website are setup to this, also popups as well. 

Chris - DONE. Connected all three service page forms. CoolTone, Coolsculpting and Vaginal Rejuvenation.

To connect the form on Consultation page. We need to replace it with our own form. Current form is working through third party service on which we don't have control over.
