I just tested the pop up on the 

https://elitesculptmd.com/coolsculpting-elite/ 
page and I am not seeing it. 
I am also seeing about a 5 to 10 second delay by the time the pop up shows. 
Can you please investigate?

Hector Mota, Operations Manager at Medstar Hector - That's strange. I just checked and it almost instantly after page finished loading. I also checked in this popup settings and there is no delay added to it.

My test submission from this popup came in the sheet as well.

On the row no. 349 in the below screenshot. This lead also came in from this popup successfully on the same date you tested that's why it's very interesting yours didn't came.

Chris says that we need to add a pop up bar that says the event on their website and when they click it, it opens up a pop up. 
We need to created a Coolsculpting RSVP event pop up. The kind that we may do a text blast for them.
