J - do not start this project until I give you the final direction. I am not sure if the doctor wants to do it but if he does we will need to create two gravity forms. One for men and one for women that will go the hormone page of the site. We will need to hide in an accordion so there is not a huge form. You will use the same questions found on the wellness assetments here:

I think it is a duplicate so you only have to create one for men then duplicate it for women. 

Ok create the form for males first. Have a button on the main page

and when they click on this have them taken to the page with the form and modify the url slightly.

Yes, but it will not be a popup it will be a standard form we insert on
another page.

That form looks good but lets add that on a separate page. Let's add a
green button to the mens and womens page that takes them to the separate
page. one for men and one for women. Revert the forms that were there
before. I am not sure if the drop down is easy to understand. I would add
the title above it: example:

Decline in General Well Being [title]
Please select one [drop down]
Never {drop down]
Mild {drop down}

etc.

I think there is different question on the women intake form. One of
of them asks about breast cancer? Can you double check both forms from the
orginal site please.
