For the form at the bottom. We can't update it's fields or add styles to it on a specific page to show it to client as it's built on FOOTER block which is same across all pages on the website. So if we make any change in it. It will appear on all other pages on the website. Let me know if you want me to update it on all pages.

Isaac  - can you please put the locations below the contact form? Also make the contact form look better. How are the other pages going?

Chris - As I mentioned in my previous reply. Bottom form is a part of footer block which is unique on all pages on the site. We can't update it for a specific page. If we make any change in the footer block > bottom form or add anything below it. It will automatically get added on whole website.

Please confirm if you still want me to go ahead and update it.

Finished all work on New Coolsculpting service page. Below is the link.

I was able to add unique form on the top.

In case we need to add unique form at bottom like skinney. We can add but then two forms will appear. 

One unique bottom service form and one that's in the footer.

One way to get around this issue is to hide footer form on service pages using custom code.

Once you confirm then only I will go ahead and work on other service page and confirmation pages to avoid any double work.

Nice work. Yes hide the footer on the new CoolSculpting and Miradry page and then add a second form to the bottom. Once we approve we will make the pages live.
