Shane, those numbers are both correct. but they are Callrail number pool since the client has Callrail java snippet on there. 
But the correct number is the 2615. 
BTW, I called the office and both work. 
But the one on the photo you shared is no longer showing up

 - Form was submitting the lead successfully to both client's email + leadsheet. Just wasn't getting redirected to confirmtion page due to incorrect page setup in confirmation page tab settings.
