I spoke to Dr. Chalik today and we need to change the consultation form on her website. 
Both of the side forms on this page have the word "Schedule a FREE consultation" 
However, they are not FREE so we need to remove that word out of there. 
Please remove it from both side forms on the pages below. 
This is Squarespace so I could not do it myself.
