To make the pdf's fillable we will have to create all forms from scratch again. This task would take more than a couple of hrs. Please confirm with the Client and Chris.

I have now added the second location in the footer, header, and contact us page on stamfordcenter.mysculpt.net.

On the other things you mentioned, I am waiting on Chris to respond to my email so I can then ask the client. 
I will let you know once he responds. 

On the PDF, Chris said to just add the pdf as something someone can download and print.
