I am getting an error when I test the side forms for the allergies page.
I am not seeing a thank you page..
I am also not seeing the lead populate on the leadsheet.

Here is the screenshot of the error

BTW, it appears that all side forms are having this issue..
Same on Pediatric ENT page...

, I checked all the service forms and it is all fine. I also tested one of the form and redirected to its confirmation page. To email in forms and zapier: entstaff@gmail.com. Popups are all showing as well.

Good morning.
It is working fine now.
However, that was not the case yesterday as I tested all 3 side forms. Leads were not populating on leadsheet.
Perhaps Isaac fixed it.
In any case, thank you.
We are good to go now.
