May I offer a small suggestion?
I would refrain from telling them what time is better to contact you. 
This way, they could never use that as an excuse. 
Remember, we are secret so that means we are available anytime. 
In any case, they will not call you after that time anyway since most spas close after 8PM... 
Furthermore, you are not going to pick up anyway.

I totally understand. Some forms ask me what day and time I want to schedule consult for. But I ignore all of those forms. 
I simply fill out the name, phone, email and message. And done. 
No worries, you're doing great.

Hector Mota, Operations Manager at Medstar Hector let me fill in the sheet now. But so far I got a text at 8:24 am EST, Thursday Oct 15, about a day after I submitted the lead. And no email. I'm guessing they don't have auto emails set up then? I was going to wait until Monday to write the mini-report to see if they called or followed up again.

He was on the phone with the client who seems to be complaining about the lack of quality leads.

I generally move the latest secret shop to the top.. I will show you during meeting..
Second, you left out the info on the email, name you used on the sheet. (see my sample)

Spa Response time: Text Message: Yes, 1  day later - 10-15 - 8:24 AM (EST time)
Spa Response time - Email: None.
Problems with form/button: None

Follow up Phone call: None thus far.
Follow up Phone Email: None thus far.
Follow up Phone Text: None thus far.

Areas that need improvement: No direct call. There was only one text, a day later. Also, no emails, no follow up phone calls or any other type of follow ups.

Summary: The overall response time to the secret shopper leads test from the staff at Elevate MD requires immediate attention. For one, they never called the lead directly. Second, there was only one text sent a full day later. In addition, there were no emails sent at all. Lastly, no form of follow up calls, text or email done with the lead.
