I just tested the Botox side form and I am not seeing the lead show up on the leadsheet. 
The Dermal fillers side form is also not connected. 
Can you please fix this when you get a chance?

Jonnamae Jumawan, Medstar Jonnamae Kristabel, Medstar Kristabel -  Whenever you create a new GF form using DUPLICATE option. Let's make sure we also copy the ZAPIER post URL.
It doesn't get copied over automatically when you duplicate the forms so you will have to manually copy it. All other settings should also be same in zapier tab.
