Hi Isaac Dr. Eman wants to know if it is possible to place a checkbox next to the agreement under all gravity forms on her page. She wants the client to have to check the box before submitting the form. She also wants all gravity form agreements to say:

"By submitting this form, you agree to be contacted via phone/text/email, and you agree to our office policies. "
