Just looked into it and seems like WIX guys removed it on their end as earlier there was an option to add "Contact" section on service pages in settings which is now appearing only for home and some other pages but not for booking or service pages.

This option disappears when you are editing service pages. Please see attached.

Odd anything we can do to turn it back on?

Chris - Just checked again but couldn't find any option to turn it back on. 

The only option here is to contact WIX support regarding this.

Sorry but I have no info on it.  Both form and the option to add it back again is vanished from the site dashboard.

I was able to locate the page to create a ticket on this service. I have asked their support regarding this. Waiting for their reply.

For now create an optinmonster static popup. we are moving them a new website so it just temporary.
