Okay so I have tested the pop up on mobile and on different browsers. It appears we have an error.
The URL https://gracebeautymedspa.com/gb-booking-options/botox/ is
But I am getting the same results for the other pages =

Hector - I just checked for it on my end and got redirected successfully to Confirmation page - https://gracebeautymedspa.com/c/botox-westlake/ after filling the popup.

Please try again in incognito mode or some other browser. Let me know if you still see the issue. Thanks.

Isaac, I generally test in Private mode since it is the best way to get the popups all the time. 
I just checked it now and it is finally working properly. I see the confirmation page.
