Hi J, we need to set up three new popups for Grace Medical Aesthetics. One for women, one for men and one for both genders. They already have a zapier account set up so you can use the one they already have. 

Large Type: "Schedule a Complimentary Hair Restoration Consultation"
Small Type: "Please enter your information in below:"

There is no thank you page on these so just have the pop close.

We have the optinmonster code added so you don’t need to activate them on the site. Just turn them on. Also can you add an additional bcc email to grace medical zapier notification:

Great, it pops right up can you add a 5 second delay on it. Also make sure
the x is visible to close the box if you do not want to fill the popup out.
