Based on my KW research, the permalink should be /orthodontist-[LOCATION] or at least /orthodontist-braces 

orthodontist | 450000 | Low

Also very few ads for orthodontists, even in states with a lot of kids. The ad market for teeth straightening is predominately made up of ads for invisible aligners from mail direct clubs. very few private practices. 

The one exception is orthodontists running specials. Of which, the standard deal is $1,000 off

This coincides with the dental industry that is predominately referral based. Meaning, SEO may be a better marketing strategy compared to ad market dominated by teeth aligners.

Smile Direct club has been a complete nightmare for the Invisalign market.  Their commercials are killing it.
