Good morning. 
I looked at this client's lead sheet and was surprise to see NO LEADS on it since you created it back on March 12th. 
I just finished testing the Coolsculpting POP Up on this https://injectiongal.mysculpt.net/c/coolsculpting/ site. 
However, I am NOT seeing the lead come into the lead sheet. 
Can you please look into this?

I am seeing that the leads time on the sheet are off when using the CS side form.
I filled out the lead at 11:55 AM EST time and look at the time stamp.
Can you please fix this?
