Currently all service pages have same ID form in the sidebar since all of them are using same template except the emsculpt page that we recently built from ground up on their website.

So in order to put different ID forms on above service pages we would need to first create custom template for them + register widget for each or we can also go other way. Create custom full width template and add the form directly thru wpbakery instead of widgets like we have done on emsculpt page.
