Hi Jonnamae Jumawan, Medstar Jonnamae will you please add another button to the menu that says 'Book Online' that uses this booking plugin code? Make sure it is on mobile and desktop.

Also, please remove the 'call us' on desktop and just keep the phone number on its own. 

You may need to put the phone number and booking code on a new line on Mobile.
Add a sticky header to the desktop with the phone number and book online button to make it less crowded.

Jonnamae Jumawan, Medstar Jonnamae - Yes. Using the custom code we can make the form appear in popup on button click but I think the easiest option here would be to create a new page and place the form there and link the menu link to this new page.
