Isaac it looks like this pop-up is on sleeknote. Can I get the credentials to update the disclaimer or will you need to do that? 

Need to include 'phone' to the part that says text/email and double check any other pop-ups that might be on this site.

I am not seeing either of these pop ups populate on the leadsheet after I ran a test.

Hector - Not sure why but it seems when you submitted the popup it didn't get submitted because I generated a CSV in the Sleeknote and your test lead is not in there and that's why it's not in the leadsheet.

After submitting it did you get redirect to the thank you page successfully on your end?

Can you please make a screenshare video of submitting it again on your end? so I can send it to the Sleeknote team to debug because all looks good in the settings on our end and can't see anything that can cause this.

Thanks for following up on this. 
So it seems that the https://go.marlowemd.com/morpheus8/ has been fixed because my pop up lead test showed up this time.
However, for the https://go.marlowemd.com/c/emsculpt-neo/ I tried twice, using different information and 2 different browsers and neither one is showing up on the leadsheet.

Hector - Try again without using the "Test" in any of the fields in the popup. It seems your second submission got flagged as a spam. I would recommend not using "Test" in any of the fields mainly when you are testing multiple popups in a row on a same website from a same IP address.

I checked in the backend and all looks good. All the popups are connected to the same webhook so if there would be any error in the webhook none of the webhook will pass the info to the sheet.

Please try again on your end without using "Test" in any of the fields and let me know how it goes. If you are still not able to see your lead in the sheet. Please make a video while doing the test submission because if it still don't work for you. I may need it to send to Sleeknote support team.

Isaac, I believe that the TEST part may have been the issue since I am seeing it now.
