Good morning. 
So I was seeing several BLANK entries in the leadsheet for this client. I checked the history and saw that none of us deleted that info.. 
I tested the form on the bottom of the home page. It appears that when you fill it out, only the DATE appears on the lead sheet. 
NO NAME, PHONE or any other info shows up... 
Can you please check into this?

Fixed the lead not showing up on leadsheet.

Jonnamae Jumawan, Medstar Jonnamae - Once you have created the new Coolsculpting pages for all locations. Please make sure you create confirmation pages for all of them and then redirect the respective CS forms to them.
