I am not seeing the pop up populate either. I waited, scrolled down, nothing.
I see it on Optin and I see the GTM tag container there so it should be working.

Thanks for taking care of this. I am seeing the pop up now. However, I am not seeing the lead type show up.

BTW, I am also seeing that they have several other pop ups on Optinmonster for different LPs and they too are not populating.

I am seeing the pop up now so thank you. 
I did not say that the lead was not showing, but the LEAD TYPE. Not sure if was supposed to be showing on the pop ups.
