On the lead import sheet for New Beauty - This also applies to Dr Lowney / Abs

A few things are not importing correctly:
Optinmonster / Zapier:
Column B: lead phone
Colum D: lead referrer: email of client is showing up instead of source url
- I also want all UTM url and parameters set up so we can see the traffic source
Column F: Lead Service Type
Column G: Lead Date: Could we have a standard date format that shows the same as gravity forms
Gravity Form:
Column B: lead phone
Colum D: lead referrer: could you figure out how to show which url they filled out the form with - also knowing the GCLID or if it came from Facebook, Instagram, etc. would be very helpful
Column F: Lead Service Type
Column G: Lead Date: how can we get the creation date on this?
Will check for source URL and entry date for leads coming thru Option popup.
Column F: Lead Service Type - We need to have a field for it in the Optin popup.
Column G: Lead Date: Could we have a standard date format that shows the same as gravity forms - Sorry but it's not doable.
