Hi J, can you set up a notification or welcome email that the client will receive once they fill out the form. Currently we have it set up to notify the website owner but we need to also notify the person filling out the form with a welcome email.

Can you update this so the send from email is 
guestservices@newbeautywellness.com - right now it says admin email.

Welcome email
The doc is attached. Can you add the content from the doc to a welcome
notificaiton for each form
Make sure the email is coming from guestservices@newbeautywellness.com
After the word hello we need to add their name they put into the field. I think this is called a merge tag. I am not sure how to do it exactly you may need to research it but it should be something like:

Hello [name], 

Test it on one form first. There should be some info online. You may need to add the field name in the admin area of the fields if it is blank.

Yes please, create two forms for the top and the bottom of each treatment

J - I did not clarify the job properly. We need two notifications for reach lead form

1. To the Website owner or a lead notification. This should be sent from guestservices@newbeautywellness.com and go to guestservices@newbeautywellness.com
2 Client Welcome email. This needs to go the client who filled out the form you need to route the email field properly so the client gets it. 
The email should be sent from guestservices@newbeautywellness.com and go to [client email] inserted from whatever email address they provided.
