Hello Alyssa   and Hector , I'm sorry about what happened with the Vampire facial popup, but I guess I really created it on the same day I created the other popups.
And also, I don't know why dermal fillers are turned off, but I've already turned it back on. 

Isaac  will you double check please that the vampire facial is connected? Thank you!

Alyssa - Yes. I just tested both the popups and the static forms on this service page and confirm. All the forms are properly connected.
