I guess they aren't getting enough leads for a decent cost for what the procedure costs ( which is 150) so im gonna ask him if he wants to allocate to Coolsculpting

I disagree with his premise. Cost/click is a useless metric.  So what should I do here?

He says he needs clicks under 5$ or it's not worth it and that's why he adjusted it. He says he wont adjust it again without telling us.

Separate accounts is ALWAYS preferred. I think the better question is why is he messing with the Hydrafacial campaign at all?

I hear that. But we shouldn't have to threaten him since we have other clients for which we create our own Google ads accounts for. 
I believe this doc is a bit arrogant and will not take kindly to being told to keep his hands off the ads. He will feel they are HIS ADS... Hope I am wrong.
