I tested it out the static form on this client's https://nusbaummedicalcenters.com/medical-weight-loss-doctor-new-jersey/ page. 
However, instead of a thank you page, I only got this - 
https://nusbaummedicalcenters.com/c/home-with-video/
which does not let the potential lead know whether or not their info was submitted correctly. Is that supposed to be that way?
