Jonnamae Jumawan, Medstar Jonnamae please double-check all confirmation pages before checking gravity form connections, there is still Vitalyc information. Delete all multiplied confirmation pages, we only need one for each service page.

Also, Make sure all pages have 3 forms: Side Form, Mobile, and Bottom Form.
