All the settings look good in the webhook. I also did a test and it successfully passed the data to the leadsheet.

Moving forward whenever you are doing a form connection test. Can you do me a favor please ON your screen recording? Share the screen recording with me if you see any issues. If all looks good you can just delete it. You can use LOOM it's very easy to record your screen using it.

Good to know Isaac Klausnser, Medstar Isaac , thanks. I will record any issues in the future. From now on, I will also refrain from using 'test' and fake phone numbers. I hadn't had any problems using either before this site. I'll come up with some other test numbers and names to input so I have a variety to choose from.
