Seems like there is a duplicate thank you page for the homepage. Please delete the one not attached to the form.

Please start working on the audit for this site? Be careful about checking mobile and desktop. A few things were missed on previous sites for mobile especially. Mobile is the most important because most users are engaging from mobile.
If you find errors, you can update yourself, please do. Just be sure that the confirmation page matches.
On a couple of more recent sites, there were phone number inconsistencies and that is another essential thing to check.
