The gravity form should say, "Request a Telemedicine Appointment"
Please add a text box to the gravity form that says, "Please let us know the best time for your appointment and the concern for which you are seeking treatment."
Can you add a new text box to the gravity form with the text that is highlighted where people can type their desired time and treatment?
Also, let's delete the section on that page that says Book a Telemedicine Appointment and just leave the part about the doctor.

Abigail Jesse, FNP-BC, MSN- Abigail Jesse has been an RN for 11 years. She started in the Pediatric ICU and went back to school to become and Family Nurse Practitioner. She graduated Magna Cum Laude, member of the Sigma Theta Tau Honors Nursing Society, and was Valedictorian of her class. During her Nurse Practitioner clinicals, she spent time in a primary care weight management clinic. After graduation, her first job as an NP was in a Dermatology and Mohs Surgery Center, where she quickly fell in love with aesthetic medicine. Abigail is Double Board Certified in Dermatology and Family Practice as a Nurse Practitioner. She has been injecting as an NP for the last 6 years and has worked under some of the most accomplished injectors in the aesthetics business. She began training for other aesthetic injectors in 2020 and co-founded her own aesthetics training company, Aesthetic Academy.
Abigail takes a natural approach to aesthetics and facial contouring. Her goal is always to make you feel like yourself, but Abigail’s favorite treatments include full face restoration with the use of lasers, neuromodulators, hyaluronic acid fillers, regenerative injectables, and bio-stimulatory agents. She works with her patients to develop a plan of care to meet patient goals as well as develop a lasting aesthetic relationship.
Katie Olewine
MSN, FNP - BC
Prior to becoming a nurse practitioner two years ago, Katie Olewine, MSN, FNP-BC, worked as a nurse for 19 years. Long interested in aesthetic medicine, Katie recently joined Santé and is eager to further her education and understanding of the field. Katie thinks that part of being healthy is having confidence in all facets of oneself. She spends her free time with her husband, daughter, and two dogs. She likes planning tropical vacations and spending time with her family and friends. She is an active school volunteer for her daughter’s school.
