Can you please create unique gravity forms and confirmation pages. If Gravity Forms is not installed let Isaac Klausnser, Medstar Isaac and he will help you out. We are reformatting all of their treatment pages based on the template Isaac Klausnser, Medstar Isaac  will create with the new CoolSculpting page.

Chris, I am noticing that these pages are not added to the Services tab. Only Wrinkles Reduction is. 
Second, I think this page  https://sightofbeauty.com/leg-veins-new/ needs a bit of more content since it is almost empty.
