Good day to you. 
Thank you for taking care of this. I see the pop ups working. 
However, I found another issue. The side forms for 
https://go.dallasskincentre.com/contact-us/
https://go.dallasskincentre.com/morpheus8/
are not populating on the leadsheet. 
I did not check the other services save for the CS elite which appears to be the only side form hooked up.

I see why I did not see them at first. Those came in about 11 minutes later. Is there a reason for such delay?
11 minutes is a long time for a lead to populate.
