Glitch in GHL - creating duplicate conversation

I have received complaints from two clients regarding duplicates in the Medstar App.  This is the first one I have seen it documented.  Can you please check and see why we have duplicates in the THRIVE MED SPA account?
When I look into these, it looks like the same conversation with the same person was broken up into four separate contacts!

As far as the Issue goes with the duplicate contacts, we ran a few tests at our end and were not really able to check for a reason why this happened. I believe the "Allow duplicate contact" might have been accidentally turned on by someone in the account. We have managed to merge the contacts into one and all the information is safe in there. 

Not to worry, should this issue happen again, please do reach out to us and we shall surely escalate this with the developers.

I checked in the audit logs and not seeing any updates made in the duplicate contacts recently so my best guess is it happened due to some temporary glitch on the GHL's end.
