Isaac I want to be sure the forms are set up correctly, when I was inspecting the newly added Botox page, I see there is a bottom mobile & bottom desktop & top form. However, when you ran your Trello on the Botox template, the 3rd form was not there so it got added back when Nolie added the new page below:

I did some testing on the ones you previously checked and didn't notice an error. I am just being diligently careful by asking you to confirm. If you confirm, please let me know that we only need one form on the bottom for the new botox page.

Gerlie I did find one error when I was testing forms. On the mobile view of the gravity forms. The 'phone' field is automatically filled with placement text, I have to back-space to enter new text. Please check that the placement text does not auto-fill on gravity forms for these pages:

Alyssa - Nice. Find. Yes. We don't need a separate form for the mobile version on this page. I have now deleted it and correctly named the forms.
