Kristabel - All new forms you have created for above pages are set to redirect users to "Kybella Confirmation" page. Please update the forms to send them to their respective service confirmation pages.

While creating confirmation pages. Please make sure we have the appointment buttons at top. Just below the "Thank you" text.

Few things:

1) Please make all confirmation pages no-indexable and no follow.
2) Add the respective page content below appointment buttons on confirmation pages.
3) For testing on forms. Let's make sure you and Jonnamae always use your own email for testing or ask Hector for that. Reason is for eg. In today only while you did tests on the forms more than 20 TEXTS were sent to client's mobile and 20+ emails in her inbox within a span of an hr. Although I can see you mentioned "Kristabel from Medstar" in the lead name still some client's many not notice it and think there maybe something wrong on the website.
Real story - One of the client even emailed us once thinking someone has hacked her website only after receiving around 10 test lead emails last year.

Please confirm Hector after updating confirmation pages so he can check and verify on his end. Thank you for all your hard work.

I think I see what happened.
Kristabel tested each form (side, bottom) for every single service page. Is that the right way to do it or should we only test one page?
At the same time, we do need to confirm all are working fine.
Let's make sure we know what the right protocol is so we can do right for the next clients.

Is that the right way to do it or should we only test one page? - We should test all pages specially when these pages have different set of elements/iframes/widgets etc on them.

For testing while I do test, I use my own email. You can quickly set a new notification in GF and then toggle b/w test and admin notification easily whenever required.
