I am not seeing any side form or pop up on this client's LP. This is what I am getting when I go to the LP in private mode https://americanlasermedspa.com/services/coolsculpting/

Isaac, I also noticing that the leadsheet does not have a LEAD NOTES column. I asked the client if they removed it, she said no. Can we add one to it?

I Kind of figured that once I saw the LP had no place to add a message. However, the contact page does. 
I also got the old URL from their leadsheet. I see that the correct one does have it.
