The page looks good. 
But the side form - I filled out the lead test and it is not showing up on the bodymorphmd lead sheet. 
Perhaps Isaac needs to connect it? Unless J. knows how to to do it?

Quick question:
Shouldn't the POP UP leads also show up on the lead sheet? If so, then we still have an issue. I just filled out a test lead using the pop up and I am not seeing it. Same lead showing up on ZAPs. 
Can you please clarify for me?

Jonnamae the graphic it is not showing and the gravity form says "emsculpt" - (( on mobile)) can you look into it?
