Thank you Isaac Klausnser, Medstar Isaac , can you please clarify-   Meredith is in the 2nd attempt stage and so that should not have triggered an end to the workflow.
Is 2nd attempt accidentally triggering an end to the workflow?

Nicol Razon, Medstar Nicol - It could be that they accidentally first moved it to any of the below stages and then moved back to 2nd attempt.

consult scheduled

Unfortunately the execution logs just shows the workflow name but not the trigger name in it. I can't think of anything else. All looks good in the settings.
