Hey J, we are going to move over the testing server to the main site. http://cma.guavajelly.tv can you please make sure the confirmation url for CoolSculpting is exactly the same as the current site. You can keep the other confirmation pages with the /c/treatment name the same for the other services. Please make sure that all the gravity form notifications, confirmations, etc work, add me and you as a bcc and test all the forms. 
Please add a schedule page and create a new gravity form, keep the same fields as usual but add a service of interest drop down. Add CoolSculpting and all the services listed under medical spa as options. Please compress the blond girl at 72dpi, the image should be under 250kb.
