Can you please take a look at it now because I do not think that this looks right the way it is now.

Isaac  - can we remove this and add a widget to the confirmation page it is getting in the way.

Looks good. But Isaac, for some reason when I filled out the form, I hit submit and nothing happened. 
No confirmation page so I could not verify widget. 
I did check the lead sheet and saw the test I did there so not sure what that is all about.

I did see the problem still persisting. Thought it may have been the need to clear my cookies but I see Shane is reporting it as well. 
Isaac, perhaps you need to PURGE the cache.
