Notes: Isaac , Elizabeth sent me this video - the form field input disappears once clicking on a different field. I also checked GHL and it doesn't look like the trigger is set for this page, or at leas for the side form. Additionally, I am unable to look at the leadsheet from any of the Medstar email accounts I have access to (including support@).

Yes. He is one of our very old client and that's why this sheet was created using Hector's personal email ID. I have now added support@medstarmedia.com to this client's sheet.

I meant the tag/service type, not the trigger. Normally it displays the opportunity/contact like how I wrote in red on the screenshot
