I just tested the side form and it is not appearing on the lead sheet.

You said you connected the other forms. But I am not seeing any leads appear when I tested the POP UP on the Emsculpt page  
Not only that, it appears that NONE of the pop ups are hooked up. 
How can this be? Can you please check?

Hector - I only connected all Gravity Forms and not OPTINS. Will connect them now.

NO POP UPS LEADS SHOWING UP - All three links you have shared are of confirmation page and we only have the optins set to show up on service pages.

Hector - Just connected all OPTINS as well on this website.
